MediSave and Insurance for Cataract Surgery MediSave may be used towards eligible cataract surgery expenses at approved medical institutions in Singapore. The amount available depends on the surgical procedure code recorded for your operation, the withdrawal limits in force on your treatment date and your available balance. MediShield Life or private insurance may contribute as well. You may still have an amount to pay yourself, so ask the treating institution and your insurer for written estimates before surgery. Yes, for eligible procedures at approved medical institutions. MediSave is a national medical savings scheme that sets aside part of your income for healthcare costs, and it may be used for eligible hospitalisation and day-surgery expenses. Cataract surgery is commonly performed as day surgery. Eligible is not the same as fully paid for. MediSave releases money up to a set limit for the operation recorded, and only against charges that qualify. Anything above that limit, or outside it, comes from insurance or your own funds. How is the MediSave amount determined? Six things decide the figure, applied in sequence. 1. The institution identifies the procedure code. Singapore uses the Table of Surgical Procedures, which gives each operation a code and a complexity ranking. Cataract extraction with an intraocular lens implant appears under codes such as SL808L, ranked Table 4A, for a unilateral procedure and SL807L, ranked Table 5A, for a bilateral one. Complex or combined operations carry different codes. 2. The ranking sets the surgical withdrawal limit. A higher-ranked table allows a larger withdrawal. 3. A day-surgery charge limit may apply separately. This covers ward, treatment, investigation and medication charges alongside the surgical limit. 4. The limits in force on your treatment date apply. Earlier published figures may have been revised. 5. Your available balance caps the release. MediSave cannot release more than the account holds. 6. The eligible bill caps it too. MediSave will not release more than the qualifying charges incurred. Ask which code will be used before comparing figures. Four amounts are easily confused: Withdrawal limit The most MediSave will release for that coded procedure Claim limit The most an insurance policy will pay for that part of the bill Actual payout What MediSave and the insurer release once the bill is assessed Final cash portion What is left with you after everything above has been applied Can a family member's MediSave be used? In some cases. CPF rules allow MediSave to be used for an approved dependant's medical expenses. The categories generally include a spouse, children, parents, grandparents and siblings, and some of these relationships carry nationality or residency conditions. Two practical points follow. The account holder must authorise it, usually through a claim authorisation form arranged by the institution. And drawing on another account does not lift the cap: the same withdrawal limit applies, funded from elsewhere. Confirm current rules with the treating institution or CPF Board before relying on this. What is MediShield Life? MediShield Life is Singapore's basic national health insurance scheme. It covers Singapore Citizens and Permanent Residents under prevailing rules and is designed for larger healthcare bills rather than routine costs. Four mechanisms shape what it pays. A deductible is a fixed amount you meet first in a policy year before payouts begin. Co-insurance is a percentage of the remaining claimable amount that stays with you. Claim limits cap what the scheme pays for each part of the bill. Pro-ration, which applies in some settings, adjusts the bill downwards before the claim is worked out. An eligible procedure is therefore seldom a fully paid one. What is an Integrated Shield Plan? An Integrated Shield Plan is a private policy sitting on top of MediShield Life: one product with two components, the MediShield Life portion and additional coverage from a private insurer. What it pays depends on the plan, the ward or facility class it covers, the care setting, the provider arrangements attached to it and your policy terms. Some policies operate a panel, a list of providers the insurer has contracted with, and pay differently outside it. Some require pre-authorisation, meaning approval sought from the insurer before treatment. Some settle directly with the institution; others reimburse you afterwards. They differ between insurers and between products from the same insurer, so only your policy and your insurer can tell you which apply. What does a rider do? A rider is an optional add-on to an Integrated Shield Plan that may absorb part of the deductible or co-insurance you would otherwise pay. It does not necessarily reduce your payment to nothing. Cover is set by the rider’s own terms, and there is a dividing line by date: requirements for new riders sold from 1 April 2026 changed, so a rider bought before then may work differently from one bought after. Ask your insurer which generation you hold and what it leaves you to pay. How does the claim and payment process work? Your ophthalmologist confirms after assessment that surgery is being considered. The institution prepares an estimated bill. The applicable procedure code is identified and MediSave eligibility is checked. Insurance benefits and any pre-authorisation requirement are reviewed. You receive an estimated breakdown of the MediSave, insurance and cash portions. You complete the required financial consent and authorisation forms. The provider submits the claim, or explains the reimbursement steps. The final bill is reconciled once the claim has been assessed. Pre-authorisation or a Letter of Guarantee may indicate expected coverage, but final payment remains subject to claim assessment and policy terms. What may still need to be paid out of pocket? Depending on your circumstances, some of the following may remain with you: • the deductible under MediShield Life or your Integrated Shield Plan • co-insurance, the percentage share retained by you • any part of the bill above an applicable claim limit • charges that are not claimable at all • a lens or implant difference, where the item chosen is not fully covered • charges arising outside the care setting or provider network your policy covers • any eligible amount above your available MediSave balance • the gap between the estimate and the final bill, if what was needed changed Which of these apply depends on your policy, the care setting and the operation performed. What should you check before surgery? Work through the estimate line by line and confirm each of the following. • Is the estimate for one eye or both? • What procedure and billing code will be used? • What amount is estimated to come from MediSave? • What amount is estimated to come from insurance? • What deductible and co-insurance apply to you? • If your policy operates a panel, is the provider on it? • Is pre-authorisation required, and who arranges it? • Is the lens or implant subject to a separate limit or exclusion, and who is charging for it? • Will the claim be submitted for you, or will you claim reimbursement? • What is the estimated cash portion? • What happens financially if the procedure or the bill changes? Every figure here is an estimate. Ask for it in writing. What documents may be needed? • identity documents • a medical claim authorisation form for MediSave, signed by the account holder • insurance policy or membership details • a referral letter, where the care pathway requires one • a pre-authorisation letter or Letter of Guarantee, a written undertaking from an insurer to the institution about expected payment • medical documents requested by the insurer • the itemised estimate Requirements vary. Ask early, so paperwork does not hold up a scheduled date. Public and private care Cataract surgery is available through both public and private providers in Singapore. Financing, referral routes, provider choice and billing can differ, and neither route is automatically cheaper, faster or better in every case. The treating institution can confirm the procedure code, the itemised estimate, whether published fee benchmarks exist for that code, and who handles financial counselling.
